Job Description:
​
Our Direct Support Professional work with individuals or small groups of developmentally disabled individuals in the community. They assist in teaching functional life skills and independent living, social, direct care and pre-vocational skills to our consumers. They serve as a liaison between the Agency and families, carry out administrative tasks related to their caseloads and may be required to drive an agency vehicle to pick up and drop off consumers.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Detail oriented, strong organizational skills, and problem solving ability.

Ability to meet deadlines, to support multiple projects, and to work both independently and as part of a team.

To provide good communications with service providers and families/advocates.

Completing all necessary documentation- Monthly Note, ISP, SCOR’s and LOC’s within required timeframes.

To meet all Medicaid billing requirements through documentation.
-
To complete semi-annual Service Coordination Observation Reports, reviewing the report and if any problems are noted, sending copies of the report to the responsible agency and ensure that appropriate action is taken.

To develop the ISP’s and complete the Service Coordination Agreements Form properly with signatures, dates and attachments.

To complete Level of Care Forms yearly or as needed.

To complete all necessary forms to ensure SSI/Medicaid eligibility.

To attend programmatic and administrative supervision.

To confirm that the service coordination activities identified in the Service Coordination Agreement are appropriate for obtaining or maintaining the person’s goals.

Review and follow up on any evidence that the personal or civil rights have been denied or abridged.

To advocate for the rights of the consumer.

To take all reasonable steps to ensure that all health, welfare and fire safety needs are met.

Serves as liaison between HOTW and individuals and their families.

At a minimum completes one face to face contact a month with each individual on caseload. At all times, the Service Coordinator must maintain program service standards.

Additional responsibilities as deemed necessary and/or appropriate by the program supervisor/ Director of Services or Administrator.

REQUIERD QUALIFICATIONS
​

A Bachelor’s Degree or An Associate’s Degree from an Accredited College or University and a minimum of one years of experience as a direct care professional. Master degree preferred in social services related area with 1 year of experience with developmental disabilities.

Strong communication skills with ability to read, speak, write and understand English and Bilingual.

Good organizational and teaching skills with ability to analyze problems and determine corrective measures.

Excellent interpersonal skills necessary to interact effectively with coworkers, employees, residents and their families.
